ADULT & MINOR AMATEUR ATHLETIC WAIVER & RELEASE OF LIABILITY

ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT

In consideration of being allowed to participate on behalf of Silverado Cup events and activities, the undersigned acknowledges, appreciates, and agrees that:

Competitors must exhibit good behavior and manners exhibited with fair play and sportsmanship. Any acts or statements which are disruptive to the smooth operations of the tournament or any division, or which are offensive, or otherwise seriously inappropriate behaviors may result in disqualification from the event. Any competitor found in possession of any alcoholic beverage, illegal drug, or narcotic substance at event site may be disqualified. All competitors are required to appear for competition at posted times. Any persons who fail to appear when his/her name is called to compete will be disqualified according to tournament rules.

Statement of Health: By my and/or Parent/Guardian's signature below I confirm that I am in good health and there is not reason why I cannot participate in this tournament and/or event. ; and, I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately;

Aand, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Silverado Cup their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law. traveling to and from and for participate in this event. In addition, I hereby now and forever, accept any and all responsibility for any actions in conjunction with this event. I also waive any compensation for the use of any pictures or videos taken of me at this event, used directly or indirectly for the promotion of future events.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY REGISTERING TO OUR EVENTS, AND REGISTERING IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

No refunds or switching names of participants. All sales are final.
